In one of the Western Sky Aviation incidents, a pilot even detected their plane hitting the runway edge lights but chose to return to Perth immediately, finding no aircraft damage. In another, a different pilot heard an unusual noise but assumed it was from inside the cabin, continuing the departure and later discovering minor damage to the right engine propeller.
The safety bureau's report sheds light on the issue, stating that in all three incidents, pilots taxied past the turn onto the centerline and lined up with the runway edge lighting on the far side. It's a complex issue, made more challenging by the runway's design. Runways often have centerline and edge lighting, and in this case, the lighting from the taxiway continued across the runway, making it harder to distinguish the centerline.
The bureau also points to the pre-dawn darkness and the runway's unmarked extended pavement areas, which made it appear wider than it was. These factors, combined with the difficulty of seeing the required runway markings at night, created a perfect storm for misaligned takeoffs.
The 2023 and 2024 incidents have led to special reminders for pilots and a request from the airport to highlight the higher-risk nature of this spot. Virgin Australia Airlines has taken action, adding caution notes to its Perth Airport information and revising pre-takeoff procedures to reduce crew workload during lineup.
The airline has also incorporated the 2023 event into non-technical skills training. A spokesperson for Virgin Australia emphasized their commitment to safety, stating, "Our team closely supported the ATSB's investigation, and we immediately took action to enhance our safety protocols."
