During the inquest, Teresa O'Sullivan, representing the coroner, praised Dr. Boros-Lavack's personalized and compassionate treatment of Cauchi. However, she highlighted a critical error: the doctor's failure to take proactive steps when Cauchi's mother raised concerns about his mental health.
O'Sullivan stated, "It was a major failing for the doctor not to urge Cauchi to resume his medication."

O'Sullivan's recommendation for an investigation by Queensland health authorities came after calls from some of the victim's families. She also identified a missed opportunity for police intervention due to an email about Cauchi's mental health being overlooked.
The inquest further revealed that a security guard, the only person in the CCTV control room during the attack, was not competent for the role. However, the shopping center has since improved its processes.
Among her recommendations, O'Sullivan called for improved mental health outreach services and short-term housing for those with mental health issues. She praised the police and emergency services' response but highlighted issues with inter-agency cooperation.
O'Sullivan also recommended a public awareness campaign with the message "escape, hide, tell" for situations involving armed offenders. She criticized some media coverage for exacerbating the trauma of victims and their families and plans to make recommendations to the Australian Press Council.
Bravery awards were recommended for the police officer who shot Cauchi, two Frenchmen who tried to stop him, Jade Young's husband, and Ashlee Good, who died shielding her baby.
Outside the court, Noel McLaughlin, Jade Young's husband, emphasized the importance of the inquest, stating, "The evidence has shown that this was not a random act but the end of a long story."
The parents of Joel Cauchi also released a statement, expressing their hope that the coroner's findings will prevent such tragedies in the future.
